Brookside Inn accepts Medicare, Medicaid, private pay, and most long term care insurances. We encourage you to tour our beautiful facility and meet with us to discuss the needs of your loved one. We have long term care, short term, post hospital rehabilitation, hospice and respite care available.

Once you have chosen Brookside Inn for your loved one we will do an assessment and will notify you to schedule a move in date. The following information will be needed to expedite the assessment process:

• History and Physical Examination (completed within the past year)

• List of Current Medications (if admitting from home medications must be in original containers)

• Copies of all Advance Directives (Medical Power of Attorney, Living Will, etc.)

• Copies of Medicare and insurance cards

• Completed ULTC 100.2 from Long Term Care Options (for Medicaid patients)
